Who Is Clinically Extremely Vulnerable (CEV)?
Individuals classified as clinically extremely vulnerable received shielding letters during lockdown. The official list of conditions that automatically put you in this group includes:
- Solid organ transplant recipients
- Active chemotherapy for cancer, radical radiotherapy for lung cancer, blood or bone marrow cancers (leukaemia, lymphoma, myeloma) at any treatment stage
- Immunotherapy or other continuing antibody treatments for cancer
- Targeted cancer treatments that affect the immune system (e.g., protein kinase inhibitors, PARP inhibitors)
- Bone marrow or stem cell transplant in the last six months, or ongoing immunosuppression
- Severe respiratory conditions (cystic fibrosis, severe asthma, severe chronic obstructive pulmonary disease)
- Rare diseases that increase infection risk (severe combined immunodeficiency, homozygous sickle cell disease)
- Immunosuppression therapies high enough to significantly raise infection risk
- Spleen‑related problems (splenectomy, sickle cell disease)
- Adults with Down’s syndrome
- Dialysis or chronic kidney disease stage 5
- Pregnancy with significant congenital or acquired heart disease
- Any person judged by a GP or hospital clinician to be clinically extremely vulnerable based on individual assessment

Underlying Health Conditions That Qualify for Earlier Vaccination
A broader set of underlying health conditions also entitles you to a higher place in the queue (group 6). These conditions make you more likely to suffer serious illness or death from COVID‑19:

- Chronic heart or vascular disease

- Morbid obesity (BMI ≥ 40)

Special Considerations for Specific Groups
Pregnancy & breastfeeding
Initially the advice was to avoid the vaccine during pregnancy until more trial data became available. From 30 December 2020, the guidance was updated to allow both the Pfizer‑BioNTech and AstraZeneca vaccines to be offered to breastfeeding women, because non‑live vaccines pose no known risk. If you are currently pregnant or planning a pregnancy within three months, discuss the risks and benefits with a healthcare professional. The calculator treats pregnancy as a reason to wait.
Under‑16s
Routine vaccination is not offered to people under 16, except in very high‑risk exposure and severe outcome cases where a full risk‑benefit assessment is performed.
